Abstract

Population aging is accompanied by an increase in multimorbidity and polypharmacy. This increases the risk of potentially inappropriate prescribing, drug interactions, and adverse events in older adults. In this context, pharmacotherapy optimization and deprescribing have emerged as fundamental strategies for improving the safety, appropriateness, and effectiveness of treatments in geriatrics.

This protocol proposes a structured, multidisciplinary approach to identifying inappropriate polypharmacy and implementing safe, person-centered deprescribing processes. It addresses practical strategies for the gradual suspension of medications, clinical follow-up, and outcome monitoring, including the emerging role of artificial intelligence and pharmacogenomics as supportive tools. The ultimate goal is to reduce unnecessary medication burden, minimize drug-related risks, and improve quality of life for older adult patients.
