Resumen

El síndrome de Sjögren primario es una exocrinopatía autoinmune de lenta progresión y crónica, que causa síntomas de sequedad debido a un infiltrado linfocitario que conduce a la destrucción de las glándulas exocrinas. Clínicamente se caracteriza por sequedad oral y ocular. Más de la mitad de los pacientes desarrollan manifestaciones extraglandulares que pueden afectar a cualquier órgano, ser la primera manifestación del síndrome y que van a determinar el pronóstico del paciente. El diagnóstico de la enfermedad suele demorarse debido a unos síntomas pobres e inespecíficos al inicio, que hacen al paciente visitar a multitud de especialistas. Presenta un impacto importante a nivel físico, social y psicológico, y un aumento de la morbilidad debido a la acumulación de problemas visuales, orales y sistémicos. La complicación más temida es el linfoma no Hodking. En los últimos años, el mayor conocimiento de la enfermedad ha inducido también mejoras en su manejo terapéutico.

Abstract

Primary Sjögren's syndrome is an autoimmune exocrinopathy of slow progression and chronic, that causes sicca symptoms due to lymphocytic infiltration that leads to the destruction of the exocrine glands. Clinically it is characterized by ocular and oral dryness. However, more than half of the patients develop extraglandular manifestations that can concern any organ, can be the first manifestation of the syndrome and which are going to determine the prognosis. The diagnosis is delayed due to poor and unspecific symptoms at the beginning of the disease, that make the patient visit different specialists. It has an important impact on a physical, social and psychological level, and an increase of the morbidity due to the large number of visual, oral and systemic problems. The most concerning complication is the non Hodking lymphoma. In the past years, better knowledge of the disease has led to improvement in its management.
